DECONTAMINATION DECLARATION FORMS
IMPORTANT! All pipettes for calibration MUST be safe to handle and accompanied by a completed and signed Decontamination Declaration so we know they are safe for us to work on. This is irrespective of whether the pipette required decontamination or not.
NOTE: The return of your pipette will be delayed if we do not receive a form with your pipette.
